QME Copilot ingests 200-2000 page medical record PDFs on your Mac and drafts a chronological Review of Medical Records with a page citation on every fact.

The Review of Medical Records is the longest part of a QME report. Now it is the shortest.
Record review is not intellectually difficult because the physician lacks judgment. It is difficult because the source material is scattered, repetitive, scanned, duplicated, and unforgiving. QME Copilot compresses the clerical burden into a cited draft so the evaluator can spend time on the medical-legal opinion.
How it works
Load the concatenated record set you already received. OCR handles scanned pages when text is unavailable.
Provider, date, encounter type, imaging, medication, surgery, and procedure facts are extracted locally.
Review every fact against its page citation. Verify, flag, or edit before it ever reaches the report.
Send the cited Review of Records into your .docx template, then edit like any report draft.
Local-first by design
Generic medical scribes are not built for California medical-legal reports. QME Copilot is a desktop application for record review. Records stay on the Mac. License checks are the only required network path.
PDFs, OCR text, extracted facts, and draft outputs remain local.
Each seat is bound to one Mac. Deactivate to move machines.
No file names, page counts, extracted content, or report text are collected.
Network traffic is limited to Ed25519 license verification.
What you get
QME Copilot turns the master PDF into structured, report-ready material you can inspect, edit, and defend — from the chronological draft to the audit trail.
Word doc, plain text, and structured JSON. Drops into your report template — or pours into your own letterhead .docx.
Every statement in the draft links back to the source page in the master PDF. Verify any claim in seconds.
Every clinician seen by the examinee, with specialty, role (treating / consulting / QME / AME), and date range.
Every visit segmented and classified — PCP, ER, ortho, neuro, pain management, PT, EMG/NCS, MRI, ESI, surgery, QME, AME — built into the chronology.
Every MRI, X-ray, CT, and EMG/NCS, with the FINDINGS and IMPRESSION blocks pulled from the original report.
Injections, ESIs, operative reports, and surgical history compiled with dates, providers, and outcomes.
Every prescription with prescriber, fill dates, dosing changes, and gaps in coverage. Pharmacy printouts parsed even when scanned.
Treating, QME, and AME opinions from the record set surfaced, attributed, and cited — so you can address them directly in your own report.
Captures what you reviewed, when, and what you flagged. Defensible record for medical-legal challenge — the trail your report stands on.
And the supporting layer: custom .docx letterhead templates, conflict and duplicate flagging across the record set, gap-in-care detection, date-of-injury identification, and the structured JSON export for any downstream tooling you build.

The product is designed to avoid cloud PHI transfer by processing locally. Your practice remains responsible for its HIPAA policies, device security, access controls, and professional compliance obligations.
No. QME Copilot produces drafts. You produce reports. The physician reviews, edits, verifies, and signs the final medical-legal opinion.
QME Copilot uses PyMuPDF for text-layer extraction, Tesseract OCR for scanned pages, LLM extraction with a rule-based fallback, and regression tests against a hand-curated harness. Current regression metrics include F1 = 1.0 on a 151-page lumbar case. The practical safeguard is that every drafted fact carries a page citation for audit.
Yes. The subscription includes custom .docx letterhead templates so exported drafts land in your report format.
The first launch is focused on California QME workflows. AME compatibility is being built into the evaluator profile and template logic.
